Colorado School-Based Clinic Locations
With 73 traditional school-based clinics (blue) and 12 school-linked programs* (purple), they collectively serve over 35,000 students annually with primary care, behavioral health, and oral health services.
*School-linked programs are smaller scale programs delivered outside of a full-service, brick and mortar school-based clinic. These programs intend to initiate services at the school, then provide a connection to a trusted community resource for medical home services, through methods such as telehealth, mobile units, or single providers.
